17-OH Progesterone (17-OH PRG)
The 17-OH progesterone test measures the level of 17-hydroxyprogesterone, a steroid hormone produced by the adrenal glands and gonads. This hormone plays a crucial role in the synthesis of cortisol, a hormone essential for regulating metabolism, immune response, and stress. Testing 17-OH progesterone is especially important for diagnosing and managing conditions related to hormone imbalances and adrenal gland disorders. Why it is important to check this marker: Monitoring 17-OH progesterone levels is essential because elevated or deficient levels can indicate disorders in cortisol production, most notably congenital adrenal hyperplasia (CAH). CAH is a genetic disorder that affects cortisol synthesis, leading to abnormal hormone production and various health issues. Early detection is crucial for managing symptoms and preventing long-term complications such as adrenal crisis, abnormal development, or fertility issues. Indications for testing: Newborn screening: To detect congenital adrenal hyperplasia (CAH) early. Suspected CAH: In infants, children, or adults showing symptoms such as ambiguous genitalia, rapid growth, early puberty, or menstrual irregularities. Infertility or menstrual irregularities: In adults to investigate hormonal imbalances affecting reproductive health. Hirsutism and acne: In women, to assess for underlying adrenal disorders. Adrenal insufficiency: To evaluate adrenal gland function in people experiencing symptoms like chronic fatigue, low blood pressure, or unexplained weight loss. Fasting requirements: No fasting required for this test. However, your healthcare provider may ask you to have the blood sample drawn at a specific time of day (usually in the morning) or menstrual cycle, when hormone levels are most stable. What can be detected: Congenital adrenal hyperplasia (CAH): A group of genetic disorders affecting adrenal hormone production. Adrenal tumors or hyperplasia: Conditions that may cause abnormal levels of steroid hormones. Hormonal imbalances: Particularly those that may contribute to fertility issues, irregular menstruation, or excessive hair growth (hirsutism). Cushing syndrome or Addison's disease: Conditions related to abnormal cortisol production, which may be linked to abnormal 17-OH progesterone levels. Identifying abnormal 17-OH progesterone levels allows healthcare providers to diagnose adrenal and hormonal disorders early, enabling timely treatment to prevent serious complications. This test is a valuable tool in both pediatric and adult medicine, especially for managing congenital adrenal hyperplasia and related conditions.
