What Is PSA (Prostate-Specific Antigen)?
Prostate-specific antigen (PSA) is a protein produced mainly by the secretory cells of the prostate gland. While it is an organ-specific marker, elevated PSA levels can occur in both malignant and non-malignant prostate conditions.
PSA and Benign Prostatic Hyperplasia (BPH)
PSA levels are often raised in cases of benign prostatic hyperplasia (BPH), a common non-cancerous enlargement of the prostate. Therefore, an elevated PSA result does not always indicate prostate cancer, especially in men with BPH.
Importance of Free-to-Total PSA Ratio (fPSA/tPSA)
Studies have shown that the percentage of free PSA (%fPSA) is significantly lower in men with prostate cancer compared to those with benign prostate conditions or normal PSA levels. The fPSA/tPSA ratio has proven useful in distinguishing between malignant and benign prostate conditions.
Enhanced Accuracy in the PSA Gray Zone
The use of the free-to-total PSA ratio improves both sensitivity and specificity in patients with total PSA levels in the diagnostic “gray zone” of 4–10 ng/mL. This helps clinicians better determine the need for further investigation, such as a prostate biopsy.
