Always Tired? Blood Tests That Help Find the Cause

Persistent tiredness usually traces back to one of five areas: low iron, an underactive thyroid, low vitamin D or B12, blood sugar imbalance, or ongoing inflammation. A blood test can usually identify which one applies, with results often available within days.

If you've been waking up exhausted for weeks, pushing through the afternoon on caffeine, or feeling like rest just doesn't top you up anymore, that's not something to ignore. Fatigue like this is a signal. The good news is that the most common causes show up clearly on a blood test, long before they become anything serious.

Why Am I So Tired All the Time?

Tiredness becomes a medical concern when it doesn't improve with sleep, lasts more than a couple of weeks, or comes with other changes like weight shifts, low mood, or muscle weakness. At that point, lifestyle tweaks alone rarely fix it, because something measurable is usually behind it.

The five most common causes seen in private testing are:

  • Iron deficiency, with or without anaemia
  • Thyroid dysfunction, usually an underactive thyroid (hypothyroidism)
  • Vitamin D or B12 deficiency
  • Blood sugar imbalance, including early insulin resistance
  • Chronic low-grade inflammation, often linked to another underlying issue

Each one affects energy through a different mechanism, which is exactly why a single test rarely tells the full story. A proper fatigue workup checks several systems at once.

The Blood Tests That Find the Cause

Full Blood Count (FBC)

An FBC checks your red and white blood cells, haemoglobin, and platelets. It's the first test for ruling anaemia in or out, one of the most common and most treatable causes of fatigue. Low haemoglobin or red cell count points toward anaemia and prompts a closer look at iron and B12 levels.

Iron Studies (Ferritin, Serum Iron, TIBC, Transferrin Saturation)

You can be iron deficient and still have a normal haemoglobin reading. This is called iron deficiency without anaemia, and it's commonly missed because an FBC alone doesn't always catch it. Ferritin specifically reflects how much iron you have stored, and low stores can drain energy well before anaemia develops. Our Iron Status Profile covers all four markers together.

Thyroid Function (TSH, Free T4)

The thyroid sets the pace for your metabolism. When it's underactive, everything slows down, including energy, mood, and concentration. TSH is the standard first-line marker, with Free T4 added to confirm the picture. Thyroid issues are common and easily managed once identified, but symptoms alone can't confirm them.

Vitamin D and Vitamin B12

Both are essential for energy production, and deficiency in either is widespread in the UK, particularly through autumn and winter when sunlight exposure drops. Low B12 can also cause tingling, numbness, or memory problems alongside fatigue, while low vitamin D is linked to general low energy and reduced immune resilience.

HbA1c (Blood Sugar Control)

HbA1c gives a two to three month average of your blood sugar, rather than a single snapshot. Both high blood sugar and the early stages of insulin resistance can leave you feeling drained, especially after meals, even before a diabetes diagnosis would apply.

Inflammation Markers (hs-CRP)

A high-sensitivity CRP test picks up low-grade inflammation that isn't always linked to a specific illness yet but can still affect how you feel day to day. It's a useful piece of the puzzle when the more common causes above come back normal.

Which Test Should I Start With?

If you also notice... Likely area to check
Pale skin, breathlessness, restless legs Iron studies + FBC
Weight gain, feeling cold, dry skin, low mood Thyroid function
Low mood, tingling in hands or feet Vitamin B12
Bone or muscle aches, frequent colds Vitamin D
Thirst, frequent urination, fatigue after meals HbA1c

If more than one of these applies, or you're unsure where to start, a broader panel is usually more efficient than testing one marker at a time. Our Nutritional Status Profile covers iron, vitamin D, B12, and FBC together, while the Metabolic Panel adds thyroid, blood sugar, and inflammation markers for a fuller picture in one test.

For anyone who wants the most complete check in a single visit, the Comprehensive+ Profile combines thyroid, iron, liver, kidney, and metabolic markers, which is often the most practical option when fatigue could have more than one cause.

What Happens If My Results Come Back Normal?

This happens often, and it doesn't mean nothing's wrong. It means the cause isn't one of the common, easily-tested ones, which narrows things down rather than ruling everything out. At that point, a GP consultation is the right next step. A GP can review your full symptom picture, consider less common causes like sleep disorders, coeliac disease, or chronic infections, and decide whether further investigation or specialist referral is needed.

When to See a GP Without Waiting for a Test

Book a GP appointment promptly, rather than testing first, if fatigue comes with:

  • Unexplained weight loss
  • Persistent fever or night sweats
  • Chest pain or breathlessness at rest
  • Blood in stool or urine
  • Fatigue lasting more than four weeks with no clear improvement

These warrant a clinical assessment first, since they may need more than a standard blood panel.

How Soon Can I Get Tested and Get Results?

Same-day appointments are available at our Slough, Neasden, and Victoria clinics, and most blood test results are returned within 24 to 48 hours. If visiting in person isn't convenient, at-home test kits cover the same markers, with a clinic or phlebotomist visit for sample collection followed by postal return to our UKAS-accredited lab.

All testing is processed under UKAS ISO 15189 accreditation, with our partner lab registered with the CQC, so results meet the same standards used across UK private and NHS diagnostics.

Frequently Asked Questions

What blood test should I ask for if I'm always tired?

Start with a Full Blood Count, iron studies, thyroid function (TSH and Free T4), vitamin D, and vitamin B12. These five cover the most common, treatable causes of persistent fatigue.

Can a blood test tell you why you're tired?

Often, yes. Blood tests can identify anaemia, thyroid dysfunction, vitamin deficiencies, and blood sugar issues, which together account for a large share of ongoing fatigue cases. If results are normal, it points toward causes outside these common categories, which a GP can then explore further.

What if my blood test results are normal but I'm still tired?

Normal results rule out the most common causes, but fatigue can also stem from sleep quality, stress, mental health, or less common medical conditions. Book a GP consultation to talk through your symptoms and decide on further investigation.

Do I need to fast before a fatigue blood test?

Fasting is only required if your panel includes glucose or a full lipid profile. Iron, thyroid, vitamin D, and B12 tests typically don't require fasting, but check your specific test's requirements when booking.

How quickly will I get my results?

Most results are available within 24 to 48 hours of your sample being processed at our UKAS-accredited lab.
