What is Transferrin?
Transferrin is a serum protein responsible for transporting iron throughout the body. It plays a central role in maintaining iron balance and supplying iron to cells for essential metabolic processes.

Transferrin Saturation and Iron Deficiency
In cases of iron deficiency, transferrin saturation is one of the most sensitive indicators of functional iron depletion. Low transferrin saturation levels suggest that there is insufficient iron available for the body’s needs, even before anaemia becomes apparent.

Liver Regulation and Iron Reserves
The liver regulates transferrin production based on the body’s iron requirements. When iron reserves are depleted, ferritin levels drop, indicating a lack of storage iron. Conversely, during inflammation or ascorbic acid deficiency, serum transferrin levels may be low even if iron deficiency is not present.

Transferrin Saturation and Genetic Conditions
In the screening of hereditary haemochromatosis—a genetic condition causing iron overload—transferrin saturation is more effective at identifying the homozygous genotype than ferritin alone.

Iron Monitoring During Anaemia Treatment
For patients with kidney disease undergoing erythropoietin therapy, treatment is effective only if sufficient stored iron is available. Monitoring transferrin saturation is the most reliable method to assess iron availability during treatment.
