What is Alkaline Phosphatase (ALP)?

Alkaline phosphatase (ALP) is an enzyme found in various tissues throughout the body, with the highest concentrations present in the liver, bones, kidneys, intestines, and placenta. It exists in several forms known as isoenzymes, which vary depending on their tissue of origin.

Clinical Importance of ALP Testing

ALP testing is commonly used in the diagnosis and ongoing monitoring of conditions affecting the liver, bones, intestines, and parathyroid glands. It provides valuable insights into both hepatic and skeletal health.

ALP Elevation in Liver Disorders

ALP levels are particularly elevated in cholestatic liver diseases, including obstructive jaundice and infiltrative liver conditions. In contrast, lower ALP elevations are typically seen in hepatocellular diseases, where liver cells themselves are damaged.

ALP and Bone Health

Elevated ALP levels are also associated with bone disorders involving increased osteoblast activity and bone remodeling. Conditions such as Paget’s disease, hyperparathyroidism, rickets, osteomalacia, bone fractures, and malignant bone tumours may all lead to significantly raised ALP levels.
