Lactate dehydrogenase (LDH) is an enzyme found extensively throughout the body’s tissues, especially in the heart, liver, kidneys, and skeletal muscles. It plays a critical role in energy production and cellular metabolism.

LDH Isoenzymes and Their Classification
LDH in the bloodstream can be divided into five distinct isoenzymes, each with unique electrophoretic properties. These isoenzymes are tetramers made from two types of subunits—heart (H) and muscle (M). The isoenzymes include two homotetramers: LDH‑1 (primarily found in the heart) and LDH‑5 (mainly found in muscles), along with three hybrid forms composed of mixed subunits.

Elevated LDH Levels and Disease Indicators
Raised LDH levels in the blood can indicate a variety of medical conditions. Extremely high levels are often linked to megaloblastic anemia, advanced cancers (disseminated carcinoma), and shock.
Moderate elevations may occur in muscular disorders, liver cirrhosis, and nephrotic syndrome.
Mild LDH elevation can be seen in conditions such as myocardial infarction (heart attack), pulmonary infarction, leukemia, hemolytic anemia, and non-viral hepatitis.
